ALL those attending sanctioned Conditioning Camps with

Sr Letter of Permission to Tryout Form - Required for
U21 & SENIOR players trying out for '
U21' teams
ELITE Notification of Tryout Form - Online form only required for
U15 & U18 players trying out for
'AAA' teams
Notification of Tryout Form - Required for
U13, U15, U16 & U18 players trying out for
'AA' teams
Note: If you are planning on Trying Out for a 'AA' team in the event you are cut from a 'AAA' team, please fill out both the Elite Notification of Tryout form and the Permission to Tryout Form
